Doug, if your machinist has enough bed length,
an option would be a custom mandrel turned down to a tight fit in the receiver and then the chamber at the very end with a transverse small bolt or set screw as a drive dog. Steady rest and tailstock center, then thread. Not ideal, but still centers everything on the bore as needed.
