Smith & Wesson Problem

A friend has a S&W Model 14 6" that locks up the action on one chamber. It only does it when "fired" double action. Single action it works okay. Has anyone ever had this happen on a S&W? Are ther any ideas as to what causes it? How can it be fixed?
