Primers and stuff.
I had a soft lot of Winchester Small pistol primers a while back. They would pierce with almost any load. In the.32 mag they were really bad, but I even had the problem with 2.7gr of Red Dot and a WC in .38 special.
I know Colt changed to a smaller diameter firing pin when they started chambering the SAA in .357 Because of the exact same problem you mention. IIRC most USFA revolvers have the old style larger firing pin. So even with your normal loads you may be getting enough flow back with the larger firing pin if the primers are a little soft.
Do you have any CCI small pistol? I would try them and I bet your problems will be long gone.
